The St. Luke’s Transfer Center is designed to be a single access point for all patient transfers to coordinate patient care at the right hospital, with the right specialist, and a coordinated care team. The Transfer Center clinical teams consult and coordinate regional ER-to-inpatient, regional inpatient-to-inpatient, or physician clinic-to-inpatient transfers at the appropriate St. Luke’s facility.
